[Python-Dev] Setup changes are checked in

  I've checked in the changes to the handling of the Modules/Setup
file as we've discussed here.

  - Setup.in is now Setup.dist
  - Setup is created by configure
  - README and other instructions, comments, and documentation has
    been updated to reflect these changes
